Put together a simple customer database registration & lookup module

Hello! We own a family entertainment center and would like to use a simple "kiosk" oriented system that allows our hourly customers to enter their customer information and their kids information into a salesforce-based database. Once entered into the system, we'd like that personal data to populate a waiver which can be signed digitally or printed. For returning clients, this database needs to have a lookup form which is only accessible to our employees at the front register.....so we can double check that we have a signed waiver in our system. Pretty simple.
